If someone is seriously injured or there is immediate danger, call 000. For urgent non-emergency Orchard Tech support call 0439 32 32 32. Do not wait for an online reply. Tell your site supervisor promptly and follow the site emergency procedures.
To record a concern, sign in to your own worker account, create a new ticket and choose Workplace safety. Use a short title such as Incident, Near miss or Hazard, followed by the location.
Include what happened or what could cause harm, the host/workplace and location, the date and approximate time, whether anyone is hurt or danger remains, and any action already taken or person told. If you do not know a detail, say unknown. Do not delay reporting while you collect details.
An incident is something that happened. A near miss is an event that could have caused harm. A hazard is a source or situation that could cause harm. You can report any of these even if nobody was injured.
Attach photos or documents only when it is safe and appropriate. Do not approach a hazard to take a photo. Include only information needed to understand and follow up the concern.
Hari Yellina currently handles Worker Support requests. The portal is not monitored continuously and an immediate reply is not guaranteed. Keep your ticket number and reply to the same ticket with updates. If the danger remains or becomes urgent, call rather than waiting.
This records a report for Orchard Tech. It does not call emergency services or replace any required notification to WorkSafe or another regulator. Staff must assess and handle those obligations separately.
